Friends of Animals Wales agrees to pay the costs of the treatment set out below on behalf of the individual as named below who is currently unable to pay for the veterinary care of their pet due to financial hardship.

Friends of Animals Wales only make payment on condition that the pet is treated by our own vets Maes Glas in Brackla Bridgend.

these costs must be repaid in full by the pets owner by minimum monthly payments of £50

By ticking the boxes below you are entering into the contract with ourselves to repay to us the full costs of your pets veterinary treatment or care received from Maes Glas.

YES I ( named above) agree to repay Friends of Animals Wales in full for the costs of veterinary treatment that my pet has recieved and been paid for by Friends of Animals Wales under this agreement.

YES I understand that failure do so may result in legal action being taken against me to recover payment and any legal costs incurred by Friends of Animals Wales in the course of securing repayment.

Repayments must be a minimum of £50 per month paid by direct debit or a method agreed by us.
